Intravascular intervention is an effective strategy for the treatment of severe carotid artery stenosis. Recent technical advances, such as flow modification, intravascular stenting, and covered stents, have led to a decrease in the incidence of complications. However, the efficacy of intravascular intervention is still unclear. The current literature suggests that symptomatic patients with significant carotid artery stenosis may benefit from endovascular management; however, prospective randomized controlled studies comparing carotid endarterectomy with carotid stenting in asymptomatic patients with significant carotid stenosis are needed. The real-time elastosonography (RTE) system is a new diagnostic tool that has been recently introduced into the clinical practice. Its main advantage is the real time-visualized evaluation of the tissue elasticity, which corresponds to the physical status of tissues. In this review, we focused on the studies using RTE for the carotid artery, and discussed the role of the elastosonography in the differential diagnosis of arteriosclerotic and malignant carotid plaque, also the contribution of RTE to the treatment of the carotid artery stenosis. The prospective studies using RTE for the evaluation of carotid plaque and especially the malignant plaque were mentioned in this review. There were 18 papers included in this review. It showed the feasibility of RTE for the carotid plaque evaluation. There are several studies reporting the feasibility of RTE in the evaluation of the carotid plaque. It was suggested that the RTE is useful for the evaluation of the carotid plaque. It may be useful not only for the evaluation of the carotid plaque, but also for the evaluation of the muscle layer in the wall of the carotid artery. It was also shown that there was a good correlation between the mean shear wave velocity calculated by RTE and that measured by conventional elastography. PMID:27349941
